TY - JOUR T1 - Moderate renal impairment and risk of bleeding with anticoagulation: Comment on “enoxaparin outcomes in patients with moderate renal impairment” AU - Minichiello T Y1 - 2012/12/10 N1 - 10.1001/2013.jamainternmed.456 JO - Archives of Internal Medicine SP - 1718 EP - 1720 VL - 172 IS - 22 N2 - Dosing of warfarin and unfractionated heparin is complicated, requiring frequent assessment of coagulation variables and dose titration. Low-molecular-weight heparins boast a more predictable dose-response effect and hence freedom from routine laboratory monitoring. Similarly, the new oral anticoagulants are administered at a set dose, with no need for routine laboratory monitoring and dose adjustment. Ideally, the more reliable anticoagulant effect and ease of dosing would lead to fewer bleeding complications related to overanticoagulation with these agents.
