The report by Boult et al1 titled “The Effect of Guided Care Teams on the Use of Health Services” in the March 21, 2011, issue of the Archives concluded that the addition of guided care services within a primary care practice setting did not reduce health care utilization with 1 exception: home health care.1 However, is a physician office-based nurse optimally situated to reduce avoidable emergency department (ED) visits and hospitalizations? Would a nurse who intervenes in the home environment have greater impact?
