Rastegar objects to the term AIDS (acquired immunodeficiency syndrome) specialist in my citing the article by Kitahata et al.1 A close reading of the text shows that my term specialist actually refers to the study of Kitahata et al and those of Turner et al2 and Markson et al.3 There is no universally accepted definition of AIDS specialist, and caring for just over 5 patients with AIDS may not be enough to allow a generalist to keep up with ever-changing therapeutic recommendations. However, those patients cared for by Kitahata and coworkers' specialist group had a median survival time similar to the longest reported in clinical trials.
