In the article titled “Frequency of Failure to Inform Patients of Clinically Significant Outpatient Test Results” by Casalino et al, published in the June 22 issue of the Archives (2009;169[12]:1123-1129), David O. Meltzer, MD, PhD, should have been included in the “Author Contributions” section on page 1128 for study concept and design; analysis and interpretation of data; and critical revision of the manuscript for important intellectual content.
