RT Journal A1 Minichiello T T1 Moderate renal impairment and risk of bleeding with anticoagulation: Comment on “enoxaparin outcomes in patients with moderate renal impairment” JF Archives of Internal Medicine JO Archives of Internal Medicine YR 2012 FD December 10 VO 172 IS 22 SP 1718 OP 1720 DO 10.1001/2013.jamainternmed.456 UL http://dx.doi.org/10.1001/2013.jamainternmed.456 AB Dosing of warfarin and unfractionated heparin is complicated, requiring frequent assessment of coagulation variables and dose titration. Low-molecular-weight heparins boast a more predictable dose-response effect and hence freedom from routine laboratory monitoring. Similarly, the new oral anticoagulants are administered at a set dose, with no need for routine laboratory monitoring and dose adjustment. Ideally, the more reliable anticoagulant effect and ease of dosing would lead to fewer bleeding complications related to overanticoagulation with these agents.
