Protopathic bias was briefly considered as an explanation for the initial risk1; otherwise, the clinical significance of the lack of later risk was not addressed in either the main article1 or the accompanying commentary.2 This is surprising because the finding actually implies possible cardiovascular safety of antipsychotic medication in dementia. After all, if antipsychotic medication truly elevates cardiovascular risks, the effects should be uniform across time or cumulative, rather than only during the initial phase of treatment.
