We believe that the most likely reason why predialysis nephrology care does not appear to have a greater impact on patient survival1- 3 is that the cutoff time they chose to distinguish between late and early referral—within 3 months of dialysis start or before that time, respectively—is inadequate to assess any impact on hard end points.
