Medical records were selected using the National Claims History File to identify Medicare Prospective Payment System hospital claims with principal diagnoses believed to possibly be caused by H pylori infection or NSAID use. These diagnoses included duodenal and gastric ulcer. Gastritis and duodenitis codes were omitted. The top 10 International Classification of Diseases, Ninth Revision, Clinical Modification12 codes in the 5-state sample are noted in Table 1.
